Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. NINE, being duly sworn, deposes and says: That she is the owner of premises at 135 Oak Place, iMattituck, New York (Suffolk County Tax Map No. District 1000, Section 142.00, Block 01.00, Lot 011.000). That her husband, Joel H. Nine acquired title to this parcel on March 21, 1933 and upon his death on February 24, 1978, title was conveyed to her !along with her children, Raymond F. Nine, Marilyn N. Jessup and 'iBeverly J. Nine from Raymond F. Nine, as administrator of the llEstate of Joel H. Nine by deed dated December 6, 1978 !and which was recorded in the Suffolk County Clerk's Office on 11, 1978 in Liber 8547 Page 530. On December 6, 1978 title was subsequently conveyed to her name by deed recorded in Liber 8555 Page 599. Located on the property is a two story frame dwelling in good i condition and a 20' x 20' detached garage and 6' x 15' storage shed.
