I am the owner of premises located at 15 Youngs Avenue, Mattituck, New York. I purchased same from the former owners, Bruce and Maria Nienstadt. At the time of my purchase, there existed a shed consisting of a concrete floor and cinder block walls in the rear of the premises. Mr. and Mrs. Nienstadt indicated that the shed had been in existence since the time they owned the property. 2. Furthermore, during my ownership of the premises, my next door neighbors, Mr. and Mrs. Harry Tuttle, who have resided at 115 Youngs Avenue, Mattituck, New York, for the past twenty-five (25) years, have informed me that the structure in question was built with the original dwelling and has been in existence prior to 1957. 3. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of complefed site plan requirements where applicable. B.
