A. This application must be filled in by typewriter OF, ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form)
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
3. For existing buildings (prior to April 9, 1957) non -conforming uses, or buildings and
"pre-existing" land uses:
L. Accurate survey of property showing all pronerty lines, streets, building and
unusual natural or topographic features.
z. A properly completed applicat:pn and a consent yo i spect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.

on Ninth Street, Greenport, more commonly known as "Cherry Lane Farm", since she
and her late husband purchased the property in 1968.
5. That to the best of my knowledge, the main house, the gate house, the
garage (caretaker's) apartment, and the northwest cottage on the property have been
continuously occupied since the property was purchase by the Nelsons in 1968 until
the present.
6. That the main house has been occupied continuously by Ann Marie
Nelson since the property was purchased in 1968 until the present.
7. That the gate house was occupied continuously since the purchase of
the property in 1968. From 1968 - 1970, Tom Neville resided at the gate house.
Dorothy Frank resided there from 1970 -1971. Since 1971 until the present, family
members and friends have resided at the gate house.
8. That the garage (caretaker's) apartment was occupied continuously
since the property was purchased in 1968. From 1968 when the property was
purchased by the Nelsons, until 1970, David, a Franciscan Brother on leave from the
Brotherhood, resided in the garage apartment. From 1970 -1976, Chris Nelson
resided in the garage apartment. From 1977 -1981, Robert Patrick Nelson Sr. resided
in the garage apartment. From 1982 - 1990, William Wetmore resided in the garage
apartment. From 1990 until present time, the garage apartment has been occupied by
Robert Patrick Nelson, Jr.
9. That the northwest cottage has been occupied continuously since the
property was purchased by the Nelsons in 1968. In 1968 - 1983, Cassie Bouton, the
sister of Ann Marie Nelson, resided in the northwest cottage year round. Cassie
Bouton bred show dogs and was a regular client of Dr. Zitek of Southold. From 1983 -
1994, June Roche, daughter of Cassie Bouton, resided at the northwest cottage.
From 1994 until present, close family friends of the Nelsons, Steven Bondarchuk and
Cheryl Victoria, have resided continuously at the northwest cottage.
10. That your deponent makes this affidavit knowing that its contents will be
relied upon by the Southold Town Planning Board in approving the minor subdivision

2. That your deponent makes this affidavit in support of Ann Marie Nelson's
application for a minor subdivision of her property located on Ninth Street, Greenport.
3. That I have known Ann Marie Nelson for over years.
4. That I have been familiar with the property owned by Ann Marie Nelson
on Ninth Street, Greenport, more commonly known as "Cherry Lane Farm", since she
and her late husband purchased the property in 1968.
5. That to the best of my knowledge, the main house, the gate house, the
garage (caretaker's) apartment, and the northwest cottage on the property have been
continuously occupied since April 1957 until the present.
6. That the main house has been occupied continuously by Ann Marie
Nelson since the property was purchased in 1968 until the present. Chris Toner
occupied the main house continuously prior to April 1957 until the property was sold
to the Nelsons in 1968.
7. That the gate house was occupied continuously from April 1957 until the
property was purchased by the Nelsons in 1968 and has been occupied continuously
since the purchase of the property in 1968. From 1968 -1970, Tom Neville resided at
the gate house. Dorothy Frank resided there from 1970 -1971. Since 1971 until the
present, family members and friends have resided at the gate house.
8. That the garage (caretaker's) apartment was occupied continuously
since April 1957 until the present. Prior to April 1957 until the property was purchased
by the Nelsons in 1968, the garage (caretakers) apartment was occupied continuously
by an artist residing there. From 1968 when the property was purchased by the
Nelsons, until 1970, David, a Franciscan Brother on leave from the Brotherhood,
resided in the garage apartment. From 1970 -1976, Chris Nelson resided in the
garage apartment. From 1977 -1981, Robert Patrick Nelson Sr. resided in the garage
apartment. From 1982 - 1990, William Wetmore resided in the garage apartment.
From 1990 until present time, the garage apartment has been occupied by Robert
Patrick Nelson, Jr.
9. That the northwest cottage was occupied continuously after April 1957
until the property being purchased in 1968 by Chris Toner's daughter and has been
occupied continuously since the property was purchased by the Nelsons in 1968. In
1968 - 1983, Cassie Bouton, the sister of Ann Marie Nelson, resided in the northwest
cottage year round. Cassie Bouton bred show dogs and was a regular client of Dr.
Zitek of Southold. From 1983 - 1994, June Roche, daughter of Cassie Bouton,
resided at the northwest cottage. From 1994 until present, close family friends of the
Nelsons, Steven Bondarchuk and Cheryl Victoria, have resided continuously at the
northwest cottage.
10. That your deponent makes this affidavit knowing that its contents will be
relied upon by the Southold Town Planning Board in approving the minor subdivision
of property known on the Suffolk County Tax Map as 1000-45-6-9.
