This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. . 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of p~'operty showing all property lines, streets, buildings and unusual natural or topographic featu res. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Rev. 10-10-78 STATE OF NEW YORK) COUNTY OF SUFFOLK) .SS: MARY NAWROCKI, residing at (No #) Old Main Road, Mattituck, New York, being duly sworn deposes and says that: Deponent is the owner of certain premises on the North side of Old Main Road, Mattituck, New York as shown on the survey attached hereto. That deponent and her brother, FRANK NAWROCKI purchased the premises on June 13, 1952 from ROSARIO OLIVO. Frank Nawrocki died on January 18, 1961 and thereby the premises became owned by deponent individually. That on the date of purchase of the premises, there were two buildings, the residence and the garage, as shown on the survey attached. That no additions or alterations have been made to the premises since that time that would require a certificate of occupancy. Deponent makes this application to the Building Department of the Town of Southold for a Certificate of Pre-Existing Use for both the residence and the garage. before me this 1985.
