APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(5-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building,
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.

HOUSING CODE INSPECTION

Upon receipt of an application for a Pre -Existing Certificate
of Occupancy, I made an inspection of the buildings on this prop-
erty. Accompanied by Mr. Smith, I began this inspection at ap-
proximately 9:00 A.M.
The dwelling is a wood framed building constructed on a
brown stone foundation that provides a partial cellar; balance
of foundation is crawl space. First floor consists of two rooms
at rear of building, one of which provides rear entry and a
secondary stairway to second floor; a kitchen and an enclosed
porch on east side, porch provides an entry, also access to cellar;
dining room; living room with hallway for northwest entry; living
room on northeast corner; and a front entry hallway containing
stairway to second floor. Second floor has five bedrooms and a
full bathroom. Bedroom at south end of second floor has access to
secondary stairway. Roofed over front porch protects front entry.
Accessory structures include a large barn with loft, a
three -car garage, one and one-half car garage, storage shed,
chicken coop, and one -car garage. All accessory buildings are
wood framed.
The following violations of the Housing Code, Chapter 52,
Town of Southold, New York, were found:
DWELLING
Heating - no central heating system, no means of heating
rooms, Article V, Section 52-53 A 1.
Kitchen - no cooking and refrigeration facilities, Article V,
Section 52-57 A 1. Ceiling plaster loose and missing,
Article III, Section 52-32 C.
Porch - east side, entrance door damaged, does not provide
mate protection to building, Article III, Section 52-31 B.
Dining room - plaster walls, cracked and loose around thimble
of chimney, Article III, Section 52-32 C.
Living room - closet under hall stairway, holes in plaster
ceiling and walls, plaster loose and cracked, Article III,
Section 52-32 D.

Stairway - prime and secondary from first floor, no light
to aim safe ascent or descent, Artible II, Section 52-26 B 3.
Stairway from entry hallway, railing loose and shakey,
Article III, Section 52-30 A.
Hallway - second floor over stair landing, hole in plaster
ceiling, loose plaster, Article III, Section 52-32 C.
Bedroom - northwest, plaster walls, cracks and holes in
plaster, Article III, Section 52-32 C.
Bedroom - south end, broken window pane, Article III, Section
T2__ -_31-C. Ceiling light fixture mounted on wainscoating,
wires from fixture are exposed, Article V, Section 52-56 A 1.
Exterior - roof and cornice at rear of building over rear
entry door, trim and roofing rotted away leaving large
opening, Article III, Section 52-30 C, Section 52-31 C.
Hot water heater - gas fired, located in cellar, not vented
to chimney, Article V, Section 52-53 H 3.
Foundation - excessive openings in crawl space, Article III,
Section -31 A.
Porch - at front of building, steps uneven and deformed,
Art cle III, Section 52-30 B. Center posts supporting roof,
loose and shakey at base, Article III, Section 52-30 A.
ACCESSORY STRUCTURES
Three -car garage - shingles missing from roof, several broken
window panes and a missing window sash, siding loose and
deteriorating at some spots, Article III, Section 52-30 B
and 52-31 B, C.
One and one-half car gara e - no garage door, openings in
roof and sidings; south side lean to, wall missing, building
deteriorating, Article III, Section 52-30 A, B, C and
52-31 B, C.
Barn - light switch box requires plate, box not securely
mounted, panel box throw off switch not securely fastened,
Article V, Section 52-56 A 1, 2.
Storage Shed - on posts, deteriorating, openings in siding
an roof, Article III, Section 52-30 A, 52-31 A, C.
One -car ara e - two broken window panes, Article III,
Section 5 -31 B.
