Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposa!. (S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1057), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey.of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings, 3. That she is the owner of premises situated at Oregon Road, Cutchogue, New York 11935. That said premises have been owned by your deponent since October 29, 1970, and prior to that by deponent's family since the early 1920s. That since April 23, 1957 there have been no structural alterations to the dwelling. That no changes to the premises or their use have been made since April 23, 1957, the date of the adoption of the Southold Town Zoning Ordinance. That annexed hereto and made a part hereof is a survey of the premises prepared by Donack Associates, dated the 5th day of September, 1984. This Affidavit is made to induce the Town of Southold, through its appropriate officials, boards or employees, to issue a certifi- cate of occupancy, knowing and intending that the said Town, its officials, boards or employees will rely upon and be induced to act by reason of the representations made herein.
