APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:

Upon request of the Southold Town Building Department,
I made inspection of thus one and one half story framed
dwelling and found the following violations of Local Law #1,
Housing Code, Town of Southold. I picked up key from the
RHssell P. Silleck Agency, Main Road, Cutchogue, and began
inspection at approximately 11:00 a.m.
FIRST FLOOR
Kitchen: Southeast entrance, no electric switch or
means of controlling light in room at entry - Section
529b.
Living Room: West side, entrance from screened porch,
no electric switch or means of controlling light in
room on entry - Section 529b.
Bedroom: North end of building, east wall, plaster
cracked and loose - Section 303c.
SECOND FLOOR
Stairway: No light to allow safe ascent or descent -
Sectio 215c.
Half story - unfinished attic
Cellar: Partial, broken window pane - Section 302b.
Light socket not securely fastened to fixture - Section
528a. Crawl space under north and south bedroom sections
of building and living room section; access to this area
and vent sections are open and subject to elements,
insects, vermin - Section 302a.
House vent on east side, vertical pipe out of ground
requires cap - Section 504b.
Electric service enters building from garage, fuse box
in crawl space, north end, on east wall box deteriorating -
Section 528a, c.

Building unheated - occupancy from May lst to October 31st
only - Section 513a.
Inspection completed at approximately 11:40 a.m., key
returned to Russell P. Silleck Aaencv.
