This application must be filled in by typewriter or ink and submitted to the Building Department with the following:
A. For new building or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or
topographic features.
2. Final Approval from Health Dept.of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 1%lead.
5. Commercial building, industrial building,multiple residences and similar buildings and installations, a certificate
of Code Compliance from architect or engineer responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.

Dear Sir:
The above referenced project was inspected on September 21, 1998, at the request
of Mr. Robert Harrington. The purpose of the inspection was to determine if the as-built
roof construction, primarily in the cathedral ceiling of the Gathering Room, falls within
the allowable spans of The New York State Building Code.
The as-built construction consists of 2"x10"rafters spaced at 16"on center, using
Douglas Fir Larch#2 with Fb=1,450 psi minimum, which was a field change from the
planned TJI rafters. The actual maximum clear span, measured along the horizontal is
15'-9".
It is my professional opinion that the as-built construction provides for adequate
support of the anticipated loads of the region. I believe the as-built construction is in
conformance with the general intent of the New York State Building Code.

This document provides the Federal Emergency Management Agency's determination regarding a request for a Letter of Map Amendment for the property
described above. Using the information submitted and the effective National Flood Insurance Program(NFIP)map,we have determined that the structure(s)on
the property(ies)is/are not located in the SFHA,an area inundated by the flood having a 1-percent chance of being equaled or exceeded in any given year(base
flood). This document amends the effective NFIP map to remove the subject property from the SFHA located on the effective NFIP map;therefore,the Federal
mandatory flood insurance requirement does not apply. However,the lender has the option to continue the flood insurance requirement to protect its financial risk
on the loan. A Preferred Risk Policy(PRP)is available for buildings located outside the SFHA. Information about the PRP and how one can apply is enclosed.
This determination is based on the flood data presently available. The enclosed documents provide additional information regarding this determination. If you
