2. That this Permit is valid for a period of 24 mos. wkida is corn;tiered to be the

for an extension may be made to the Board at a later date.
3. That this Permit should'be retained iodefkLitely, or as long as the said Per, nit:tee wishes
to maintain the srmcmze or project involved, to provide evidence ro anyone concerned that auth-
orization was originally obtained,
4. That .the work involved will be subject to the inspection and approval of the Board or
its agents, and noa-compgance vAth the provisions of the originating application, may be cause for
revocation of this Pemxit by resolution of the said Board.
5. That there will be no unreasonable interference with nav/gadon as a result of the work
herein authorized.
6. That there shall be no interference with the right of the public to pass and repass along
the beach between high and low water marks.
7. That ff future operations of the Town of Southold retlulre the removal and/or alterations
in the location of the work herein authorized, or ile, in the opirdon of the Board of Trustees, tile
work shall cause unreasonable obstraclaon to free navigation, the said Permittee will be re*:luire~
upon due notice, to remove or alter fids work or project herein stated without expenses to the Town
of Southol&
8. That the said Board will be notified by the Pem2ktee o~ the completion of the work auth-

9. That the Permirree will obtain all other permits and consents ghat may be required sup-
plemental to this permit which may be subject to revoke upon failure to obtain same.

Dear Mr. Levien:
The following action was taken by the Board of Town Trustees
during their regular meeting held on April 27, 1989 regarding the
above matter:
WHEREAS, Henry Raynor on behalf of John Levien applied to the
Southold Town Trustees for a permit under the provisions of the
Wetland Ordinance of the Town of SOuthold, application dated
February 25, 1989, and
WHEREAS said application was referred to the Southold Town
Conservation Advisory Council for their findings and recommendations,
and
WHEREAS, a public hearing was held by the Town Trustees with respect
to said application on April 27, 1989 at which time all interested
persons were given an opportunity to be heard, and
WHEREAS, the Board members have personally viewed and are familiar
with the premises in question and the surrounding area, and
WHEREAS, the Board has considered all the testimony and documentation
submitted concerning this application, and
WHEREAS, the Board has determined that the project as proposed will
not affect the health, safety and general welfare of the people of
the town,
~OW, THEREFORE BE IT
RESOLVED that Henry Raynor ON BEHALF OF John Levien BE AND HEREBY IS
GRANTED PERMISSION UNDER THE WETLAND ORDINANCE TO:
construct 4' x 16' ramp, 5' x 40' floating dock, 4' x 12' catwalk,
install 4 pilings to secure floating dock and ramp. Property located
on Beebe Drive, Cutchogue, N.Y.
This permit will expire two years from the date it is signed if
work has not commenced by said date. Fees must be paid and permit
issued within six months of the date of this notification. Two
i~spectio~-s are required and the Trustees are to be notified upon
completion of said project.
Please take notice that this declaration should not be
considered a ~etermination made for any other department or agency
which may also have an application pending for the same or similar
project.
Permit will be issued upon payment of the following fees:

5' x 40'
