Pilot Plan must be drawn to scale on the reverse side of this application showing the location of property, the names and locot ons of owners of aa]oinJng properties, and a d~tai ed description of oroaosea project. No operation shall be initiated by the applicant until ail permits that are required are issued. D. A detailed statement of the rehabilitation and proposed condition of the premises after the work ~s completed must be given, including a survey ;~ required. E. ~z ~ermit is being sought for a more sizeable excavation than the construction of a private singl~ dock or jetty, then this application must be accompanied by a survey and topographical m6p certified by a registered land survey or Professional Engineer. ]'he horizontal control o~: surve'v shall be based upon an approval local coordinate system. The vertical control for elevation and sound- J,~gs shall be based upon United States Coast and Geodetic Survey and/or United States Geological Survey datum F. 4, statement must accompany the application describing any known prior operations conducted on the p,remises in question and whether any prior licenses or permits have been issued to erect structures or ~o dredge or deposit fill on said premises and whether any such permits or licenses were ever revoked or suspended by a ~ovemmental agency.
