Pursuant to the provisions of Chapter 615 of the Laws of
the State of New York, 1893; and Chapter 404 of the Laws of the
State of New York 1952; and the Southold Town Ordinance en-
titled "REGULATING AND THE PLACING OF OBSTRUCTIONS
IN AND ON TOWN WATERS AND PUBLIC LANDS and the
REMOVAL OF SAND, GRAVEL OR OTHER MATERIALS FROM
LANDS UNDER TOWN WATERS;" and in accordance with the
Resolution of The Board adopted at e meeting held on May ][.~.

To construct a 4' x 45' catwalk, 4' x 80' dock, 3' x 20'
ramp, 8' x 40' floating dock, install 40 pilings. Property
located on Knollwood Lane, Mattituck, NY
all in accordance with the detailed specifications as presen+ed in
the originating application.

TERMS and CONDITIONS

part of the consideration for the iss~,u--e of ehe Permit does understaml and proscribe to the fol-
lowing: 1. The Soulhold Town Trustees Permit must be prominently
displayed on premises effected during period of work.
1. That the said Board of Trustees and the Town of Southold are released from any ami
all damages, or rla;m, for damages, Of s~s a~ing directly or indlrec~ly as a re. Ir of any oper-
ation pedotmed pursuant to this permit, ~td the said perm~t~t~ will. at his or her own expeme,

2. That dais Permit is valid for a period of 24 mos. Which is co~sidex~l to be the
estimated time required to complete the work involved, but should clrcumsrancz$ warrant, request
for an exrenslon may be made to the Board ar a later date.
3. That this Permit should be retained indeflaitely, or as long es the said Permlmm ~dsbe$
re maintain the structure or project involved, to provide evidence to anyone concerned that auth-
orization was orlglnnlly obtained.
4. That the work involved will be subject to the inspection and approval of the Board or
iu agents, and non-compliance with the provisions of the originating applicadon~ may be cause
revocation of this Permit by resolution of the said Bo~rd.
5. That there will be no unreasonable interfece~ce with navigation as a result Of the work
herein author/zed.
6. That there shall be no invarference with the fight of the public to pass and ~epass along
the beach between high and low water marks.
7. That if future op~ratinns of tbe Town of Sonthold require the removal and/or alterations
in the location Of the work herein anthorized~ or if, in the opinion Of the Board of Tmsteeg the
work ~hnl! cause um'easonable obstrocnon to free navigation, the said portal,zee will be req-ai~
upon due notice, w remove or alter dds work or project herein stated without expenses to the Tow~
Of Southold.
8. That the said Board will be notified by the Permittee or the completion Of the work anth-

Re: Application No. 655 Martin Kruse
Dear Mr. Costelto:
The following action was taken by the Board of Town Trustees
during their regular meeting held on May 31, 1989 regarding the
above matter:
WHEREAS, Costello Marine Contracting on behalf of Martin Kruse
applied to the Southold Town Trustees for a permit under the
provisions of the Wetland Ordinance of the Town of Southold,
application dated August 16, 1988 and
WHEREAS said application was referred to the Southold Town
Conservation Advisory Council for their findings and recommendations,
and
. . WHEREAS, a public hearing was held by the Town Trustees with respec%
to said application on May 31, 1989 at which time all interested
persons were given an opportunity to be heard, and
WHEREAS, the Board members have personally viewed and are familiar
with the premises in question and the surrounding area, and
WHEREAS, the Board has considered all the testimony and documentation
submitted concerning this application, and
WHEREAS, the Board has determined that the project as proposed will
not affect the health, safety and general welfare of the people of
the town,
NOW, THEREFORE BE IT
RESOLVED that Costello Marine Contracting ON BEHALF OF Martin Kruse
BE AND HEREBY IS GRANTED PERMISSION UNDER THE WETLAND ORDINANCE TO:
Construct 4' x 45' catwalk, 4' x 80' dock, 3' x 20' ramp, 8' x 40'
floating dock, install 40 pilings. Property located on Knollwood
~Lane, Mattituck, N.Y.
This permit will expire two years from the date it is signed if
work has no~ commenced by said date. Fees must be paid and permit
issued within six months of the date of this notification. Two
inspections are required and the Trustees are to be notified upon
completion of said project.
Please return to the Building Department for a determination on
the need for any other permits which may be required for this project.
Permit will be issued upon payment of the follpwing fees for
work to be done below Mean High Water Mark:
$318.00 4' x 53' dock
90.00 3' x 20' ramp
480.00 8' x 40' floating dock
500.00 20 pilings
$1388.00 Total fees
