This application is to be completely filled in by typewriter or ink and submitted to the Town Clerk in quadruplicate, accompanied by writter' permission from the owner of the property if not the same os the applicant. B. Pilot Plan must be drawn to scale on the reverse side of this application showing the location of property, the names and locations of owners of adjoining properties, and a detailed description of proposed project. (Note: Copies of the Pilot Plan may be mode by o copy machine and attached to the Permit Application.') ~ ___ C. No operation shall be initiated by the applicant until all permits that ore required are issued. D. A detailed statement of the rehabilitation and proposed condition of the premises after the~ work is completed must be given, including a survey if required. E. If permit is being sought for a more sizeable excavation than the construction of a private single dock or jetty, then this epplication must be accompanied by e survey and topographical map codified by a registered land survey or Professional Engineer. The horizontal control af survey shall be based upon an approval local coordinate system. The vertical control for elevation and sound- ings shall be based upon United States Coast and Geodetic Survey and/or United States Geological Survey datum. F. A statement must accompany the application describing any known prior operations conducted on the premises in question and whether any prior licenses or permits have been issued to erect structures or to dredge or deposit fill on said premises and whether any such permits or licenses were ever revoked or suspended by a governmental agency. G. A filing fee of $J~5'.00 shall accompany the application. This filing fee includes one inspection: there is a $5.00 fee for each additional inspection. H. On 22 August 1985, our office received an unsigned draft permit because you were in objection to the Department of States (DOS) Coastal Zone Management recommendations. In accordance with Coastal Zone regulations contained in 33 CFR 325.2(b)(2)(ii) the District Engineer cannot issue a permit unless the Department of State (DOS) concurs with an applicants coastal management certification statement. Since you do not agree that DOS recommendations should be made part of your permit, and in order to process this application in a timely manner, your application is being withdrawn. Processing will be reinstated when DOS certification is received for your proposal. The action taken at this time will in no way adversely affect your future request or any other applications you may submit for work in waters of the United States.
