This application is to be completely filled in by typewriter or ink and submitted to the Town Clerk in quadrupli.cate, accompanied by written permission from the owner of the property if not the same as the applicant. B. Pilot Plan must be drawn to scale on the reverse side of this application showing the location of property, the names and locations of owners of adjoining properties, and a detailed description of proposed project. (Note: Copies of the Pilot Plan may be made by a copy machine and attached to the Permit Application.') C. No operation shall be initiated by the applicant until all permits that are required are issued. D. A detailed statement of the rehabilitation and proposed condition of the premises after the work is completed must be given, including a survey if required, E. if permit is being sought for a more sizeable excavation than the construction of a private single dock or jetty, then this application must be accompanied by a survey and topographical map certified by a registered land survey or Professional Engineer. The horizontal control of survey shall be based upon an approval Ioca,I coordinate system. The vertical control for elevation and sound- ings shall be based upon United States Coast and Geodetic Survey and/or United States Geological Survey datum. F. A statement must accompany the application describing any known prior operations conducted on the premises in question and whether any prior licenses or permits have been issued to erect structures or to dredge or deposit fill on said premises and whether a,ny such permits or licenses were ever revoked or suspended by a governmental agency. G. A filing fee of $25.00 shall accompany the application. This filing fee includes one inspection., there is a $5.00 fee for each additional inspection. H. It has been determined that the proposed modifications will not substantially change the scope of the permitted actions or the existing permit conditions. Therefore, the permit is amended to authorize: Placement of 150 cu. yds. of dredge spoil above mean high water at a 50' x 60' area located at north end of Cedar Ave. Final grade will be increased a maximum of 2' above existing grade. There shall be no disturbance %o existing tidal wetlands ad3acen% to spoil site. This letter is an amendment to the original permit and as such, shall he posted at the Job site. RESOLVED to approve the application of Goose Bay Civic Association, Inc. to maintenance dredge Goose Creek channel,the length of the dredged area to be 1,O00 feet and the width to be 50 feet with a maximum amount of material to be dredged to be 3,000 cubic yards, subject to the following: The beach grass is to be set aside and replanted along the creek after the spoil has been deposited on the upland. 2. At the Trustee meeting held on March 1, 1983 this application was approved, with the provision that you conform to the D.E.C. permit that has been issued (%10-81-0171) in that 150 cu. yds. of dredged spoil is maintained instead of 3000 cu. yds. It is stated on the Notice of Action, if payment is not received ~n ninety (90) days, the person will have to reapply. Since this is the case, and no Town Permit has been issued, if in the future you plan to do this work, it will be necessary to reapply.
