This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. j B. C. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3 Date of any housing code or safety inspection of buildings or premises, or other pertinent in- formation required to prepare a certificate. Mulvaney Unoccupied Upon receipt of an application for a Pre-existing Certificate of Occupancy, I made an inspection of this two-story, partial one- story wood framed dwelling. I was admitted to building by Mrs. Gilbert Amiaga who accompanied me during this inspection which began at approximately 10:00 A.M. The first floor consists of a rear entry porch which is used as a laundry room, a toilet room, kitchen, dining room, living room, and bedroom. The second floor has four bedrooms, and a full bathroom. The westerly portion of this building is constructed on a poured concrete foundation which contains a full cellar. The balance of structure is constructed on piers and is tightly skirted. This section of b ilding at the first floor level has an open porch on the nortY ast and south sides. There is no central heating system in the building. A gas-fired free standing space heater is located in the living room, and a kerosene -fired, free standing space heater is located in the kitchen. The following violations of the Housing Code, Chapter 52, Town of Southold, New York, were found: Rear porch - gas laundry dryer, no shut off valve at unit, Article V, Section 52-52 B (2). Toilet room - floor covering rotted at base of toilet, Article III, Section 52-32 D. Kitchen - gas cooking range, no shut off valve at unit, Article V, Section 52-52 B (2). Living room - entry from southerly part of building, no means of controlling light on entry, Article V, Section 52-56 B (2)•. Stairway - to second floor from the first floor hallway, no light for safe ascent or descent, Article II, Section 52-26 B (3). Bedroom - second floor at westerly end of building, 4m& lamp cord outlet on baseboard of north wall, Article V, Section 52-56 A (1). Insufficient ceiling height of 5'8" in violation of the Zoning Ordinance, Chapter 100, Section 100-13, Floor Area Livable. Page 2 - Mulvaney Cellar - gas-fired hot water heater, no shut off valve at Unit', Article V, Section 52-52 B (2). Two -car unattached garage - located in rear yard area, Mingles on exterior walls are missing and in a deter- iorated state, Article III, Section 52-31 B.
