Instructions
This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5, Submit Planning Board approval of completed site plan requirements where applicable.
For existing buildings (prior to April 1957), Non~:onforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings,

AQUILO D. MUIR, being duly sworn deposes and says that skI~
resides at 690 Rocky Point Rd., East Marion, New York.
That she acquired title to herresidence pr~mises by virtue
of a deed made by Edmund T. Rackett, et ux, recorded July 14, 1941;
said premkises are now designated on Suffolk County Tax map as
1000/31/2/22.
That the dwelling located on the precmises was constructed
prior to 1957, has alsys been used as a residence and affiance
ownership thereof has always been peacable and undisturbed.
That this affidavit is made for the purpose of inducing the
building deparh,~nt, Town of Southold to issue a certificate of
pre-existing construction thereon.

I, GEO~G~ G. MUIR, residing at East Marion, New York, do hereby
make, publish and declare this to be my]ast will and testament.
lst: I hereby revoke all wills and codicils by me at any tLme
heretofore made.
2nd: I direct that my just debts and funeral expenses be paid as
soon as is practicable' after my death, and I authe~ize my co-executors
hereinafter named to make a reasonable endowment for the perpetual care
of the cemetery plot in which I am buried.
3rd: I direct that my co-executors pay out of my residuary estate
before the distribution thereof all debt, succession, transfer, estate, and
inheritance taxes levied or assessed upon or with respect to any property
whkh is included in my estate for ptrposes of computing any and all such taxes;
I further direct that my co-executors shall have no right of reimbursement
for such taxes from any person who is a beneficiary, transferee, legatee, or
divisee of any of the property upon or with respect bo which any such tax is
assessed or levied.
4th: I give and bequeath to my brother, John A. Muir, the sum of
$2,000 and all of my right, title, and interest of, in, and to the partnership
known as Muir Brothers.
5th: All of the rest, residue and remainder of my estate and property,
of whatsoever kind and wheresoever situate, I give, devise, amd bequeath to
my wife, Mary Anna Muir, absolutely and forever.
6th: If my said wife, Mary Anna Muir, should predecease me, or if
our deaths occur in or as the result of a common accident or disaster or
under such circumstances that it is difficult or impossible to determine which
shall have survived the other, I hereby give, devise and bequeath my residuary
estate as follows: one-quarter ea:h to my step-~:aughter, Susan Park U: i,
my step-sons, William Edgar Park, Jr., and David Miller Park and to my
daughter, Jill Grilling Muir in equal shares per stirpes. In the event
my daugh~r Jill Gri££ing Muir predeceases me and is single and has never
married at the time of her death, then I direct her share to be divided
equally among my surviving step-children; in the event my daughter, Jill
Griffing Muir predeceases me and at the time of her death ~ras married but
leaves no heirs then I direct her share to be divided one-half to her
husband and one-half to my surviving step-children.
~th: I nominate, constitute and appoint my ~rife, Mary Anna Muir
and my brother, John A. Muir, co-executors of this, my last ~rill and
testament, and direct that they be not required to post bond or other
security for the faithful performance o£ their duties in any jurisdiction.

publish and declare these presents as and for my last will and testament
