This grant is to be used to support the activities outlined in your proposal of August 18, 2016. A condition of this grant is that you submit a brief report by May 31, 2017 letting us know what you accomplished and how the money was spent. Payment of this grant will be made after receipt of this signed original Letter of Agreement. It is understood that any funds not used for the purposes described must be returned to the Long Island Community Foundation. This grant is subject to financial audit upon our notification during or immediately following the grant period. A separate bank account for the grant is not required, but you must maintain a separate financial record for this grant. Please acknowledge the Long Island Community Foundation in any publicity about this grant and enclose copies of any media reports along with the final report.
