This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3 Date of any housing code or safety inspection of buildings or premises, or other pertinent in- formation required to prepare a certificate. Olsen, attorney, Main Road, Mattituck, New York, I made an inspection of this one and one half story framed dwelling. I entered building at rear entrance upon instructions from Mrs. Moore and began inspection at approximately 10:05 A.M. First floor consists of a kitchen, living room, and dining room (now being used as a bedroom). Second floor has two bedrooms and a full bathroom. Heat is furnished to all rooms from an oil fired steam heat furnace located in full cellar of cement block foundation. An enclosed unheated porch provides rear entry to kitchen. The following violations of The Housing Code, Chapter 52, Town of Southold, were found: Rear porch - No means of controlling light in room at point of entry. Article V, Section 52-56 B.2. Kitchen - Plumbing leak - cold water line of kitchen sink. Article V. Section 52-51 A.2. Gas cooking range, no shutoff valve at unit, Article V, Section 52-52 B.2. Window over sink, broken pane of glass and broken storm window glass. Article III, Section 52-31 B. Living Room- Broken plaster board on south wall. Art. III, Section 52-32 C. Front entry- Door jamb, header section of frame missing. Article III, Section 52-30 C.Floor and door sill weak and springy, end of floor joist under this section of floor - rotted. Article III, Sec. 52-31 A., Sec. 52-32 A. Housing Code Inspection EH:mm -2- Richard Moore & Wf. Second Floor - Headroom, 6' 11 1/2" - violates Zoning Ordinance, Chapter 100, Article I, Section 100-13, Floor Area Livable. Bathroom - Floor covering at base of toilet, tiles missing and broken. Article III, Sec. 52-32 D. Bedroom - northwest - broken window pane. Article III, Section 52-31 B. Cellar - Outside cellar door frame rotted away leaving excessive openings for moisture and vermin intrusion. Article III, Section 52-31 A. Laundry washing machine - waste direct connection to soil line, no water seal. Article V, Sec. 52-51 C.5. - Electrical junction box on girder over furnace, no cover. Article V, Sec. 52-56 A. Heat return pipe (1 1/4" black iron) runs across cellar floor and the travel area of cellar stairway. Article I, Section 52-15, Reference to N. Y. Side window, sash does not fit window frame, leaking at sill, one broken pane of glass and two missing. Garage door, broken and missing sections of board. Article III, Section 52-30 C.
