Instructions
This application must be filled in typewriter OR ink, and submitt~to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, ~treets, and unulual
natural or topographic feature~ ,
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
For existing buil(h,gs (prior to April 1957), Non-conforming uses, .or buildings and "pre-existing"
land usus:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unuaJal natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy end condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

I, MOOREEN TERRY, residing at Orient, New York, being duly sworn,
depose and say:
That I am the granddaughter of M. ESTELLE MOORE, the present owner of the
premises located at 220 Knapp Place, Greenport, New York, and designated on the
Suffolk County Tax Map as District 1000 Section 34 Block 2 Lot 12 and am familiar
with the condition of same;
That the structures thereon have been designated, arranged and physically
occupied as a one-family dwelling;
That the condition of the existing structures on the premises are good;
That the structures are pre-existing prior to April 23, 1957 and they are
in habitable condition;
That I make this affidavit in the knowledge that the Building Department
of the Town of Southold will rely on the accuracy thereon in issuing a certificate
of occupancy.
