Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic featu res. 2. Final approval of Health Dept. of water supply and sewerage disposat--(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of p~operty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. MOODY, JR., being duly sworn, deposes and I reside at (No #) Midlothian Avenue, Fishers New York. Attached to this Affidavit is a survey dated Novem- ber 8, 1985, made by Richard A. Strauss, which accurately represents the premises as they existed on or about November 8, 1985, and prior to April 27, 1957, except for an addition as to which certificate of occupancy ~Z2348 was issued in 1966. The basis for this Affidavit is not only for the abovementioned certificate of occupancy, and letter from the building inspector date May 8, 1975, but my conversations with David Wilmerding, whose parents owned the property from the mid-40's until July of 1964 when it was sold to Robert G. Knott. Mr. Wilmerding advised me that no changes or addi- tions to the property were made from 1957 to the time his parents sold the property. This affidavit is made to induce the Town of South- old, through its appropriate officials, boards or employees, to issue a letter of pre-existing use; and knowing and in- tending that the said Town, its officials, boards or employ- ees will rely upon and be induced to act by reason of the representations made herein.
