Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new us~:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code comoliance irom the Architect or Engineer resoonsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (~rior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing ali property lines, streets, buildings and unusual natural or
topograohic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildipgs or premises, or other pertinent infoTma-
tion required to prepare a certificate.

sworn, deposes and says :-
That she is co-executor of Lillian H. Miller, deceased owner of the
pre~Lises on the west side of West Creek Avenue, Cutchogue, Town of Southold,
County of Suffolk, State of New York, which is being conveyed to Linda M.
Faulkner (Suffolk County Tax Map No. 1000 - 103.00 - 13.00 - 005.001).
That there is a 1-1/2 story frame house with detached garage on said
premises which is used as a one-family dwelling.
That attached hereto is a copy of the latest survey of said premises
dated July 26, 1966, which reflects the buildings on said premises at that
time and that there have been no changes made in the structures which would
require a Building Permit. There has been the addition of a dock.
