APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey•of property showing all property lines, streets, buildings and unusual naturol
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:

Upon receipt of an application for a Pre-Existing Certificate
of Occupancy I made an inspection of the buildings at the above
captioned premises. I was admitted to dwelling by Lillian Ovisinak and
began this inspection at approximately 1 :00 P.M.
The dwelling is a one and one half story wood framed structure
constructed on a cement block foundation. The first floor contains
a front entry hallway, full bathroom, kitchen, dining and living room
with fireplace, an enclosed jalousie porch, den, two bedrooms and a
full bathroom off one bedroom. Heat is furnished to all rooms on
first floor from an oil fired forced warm air furnace located in partial
cellar. Hot water is supplied to all plumbing fixtures within dwelling
from an electric hotwater heater in cellar. A two car wood frame
garage with work shop area is located in front yard area.
I found no violations of the Housing Code, Chapter 52, Code of
the Town of Southold, N.Y.
Inspection completed at approximately 1 : 15 P.M.

JOHN A. O'KEEFFE, being duly sworn deposes and says that he
is the attorney for the Estate of Dorothy M. Miller, late of Nassau
Point, New York, which estate is selling her home place at Nassau
Point, and for the closing a Certificate of Pre-existing Use is
needed.
The property was acquired by deed dated October 31, 1951
from Walter K. Proom to Dorothy M. Miller, recorded in Suffolk County
Clerk's office November 13, 1951 in Liber 3286 of deeds at page 556,
for the purpose of building a residence thereon and directly after
its acquisition, the house and garage were constructed, and no changes
in the exterior lines of the structures on the premises have been
made subsequent to April 23, 1957, when zoning was enacted. Hereto
annexed is a current survey of the property.
This affidavit is made for the purpose of obtaining a
Certificate of Pre-existing Use to facilitate the closing of title.
