APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:
1. Certificate of occupancy $5.00
2. Certificate of occupancy on pre-existing dwelling or land use $5.00
3. Copy of certificate of occupancy $1.00

HOUSING CODE INSPECTION

Upon request of Richard Lark, Attorney, Cutchogue, New York,
I made an inspection of this two-story wood framed building. I
was admitted to dwelling by Mr. Keith Purcell who accompanied me
during this inspection that began at approximately 4 :20 P.M.
First floor apartment with entry on south side of building
has two bedrooms, dining room, living room with fireplace , kitchen
and full bathroom. Heat is furnished to all rooms on first floor
from a gas fired hot water furnace located in cellar.
Second floor apartment with entry at front of building has
four bedrooms, living room that opens to an open porch on south
side of building, a kitchen, and a full bathroom. Kitchen, bath-
room and bedroom on east end of apartment each contain electric
baseboard heater units. Living room has a gas fired space heater.
Vent pipe has been removed for summer season, chimney connection
is provided. Cellar is partial, balance of rubble stone and mortar
foundation is crawl space. There are two gas fired hot water heaters
that furnish domestic hot water, one servicing each floor. There are
two small accessory storage buildings in rear yard area.
The following violations of the Housing Code, Chapter 52 ,
Town of Southold, New York, were found:
FIRST FLOOR APARTMENT
Gas cooking range - no shut off valve at unit, Article V,
Section 52-52 B 2 .
SECOND FLOOR APARTMENT
Gas cooking range - no shut off valve at unit, gas fired
space heater, no shut off valve at unit, Article V, Section
52-52 B 2 .
Heat - three bedrooms not furnished with heat, occupancy
permitted between May 31st and September 15th, Article V,
Section 52-53 A 1. (Amended June 8, 1972 , New York State
Building Construction Code, C. 504-1) .
Cellar - hot water heater, gas fired, for second floor, no
shut off valve at unit, Article V, Section 52-52 B 2 .
