Instructions
A. This application must be filled in typewriter OR ink, and submitted ~ aam,a,a~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer respon.sible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Greenport~ New York, being duly sworn~ deposes and say~:
That I am an attorney at law with offices at 828 Front
Street, Greenport, New York;
That I represent the present owner of the premises located
at 35 Madison Street, Greenport, New York and designated on the
Suffolk County Tax Map as District 1000, Section 40, Block 5, Lot 23
and am familar with the condition of same;
That the structures thereon have designated, arranged and
physically occupied as a one-family dwelling;
That the condition of the existing structures on the premises
are good;
That the structures are pre-existing prior to April 23, 1957
and they are in habitable condition, that I know this of my own know-
ledge having visited and viewed the premises many times;
That I make this affidavit in the knowledge that the Build-
ing Department of the Town of Southold will rely on the accuracy
thereon in issuing a certificate of occupancy.
