APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted m ~ to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal--(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of pzoperty showing all property lines, streets, buildings and unusual natural or
topograph ic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Robert Kosciuszko, being duly sworn, deposes and states as follows:
That ! am the prior owner of certain pnemises located on the east side of
Pequash Avenue, Cutchogue, New York, designated on the Suffolk County Tax
Map as District lO00, Section ]03, Block 7, Lot 029.000.
That I have reviewed a survey of Roderick Van Tuyl prepared for Cynthia
Granickas dated January 13, 1984 showing a two story frame house and
detached garage. That I was born in said house on February 25, 1947 and
can say with my own persona] knowledge that the two story frame house
and garage Were.built prior to April 22, 1957 .
That said house presently is used for residential f~m~y~purposesland
is suitable for residential purposes. That a building permit #10410Z
was issued on September 19, ]979 for an alteration to enclose the porch
in the rear of the premises and to make same a habitable area and a
Certificate of Occupancy dated October 29, 1980 No. Z10277 was issued for
said alteration. I submit this affidavit in support of an application
for a pre-existing Certificate of Occupancy for the house and garage.
