Certificate is issued is as follows: Property contains one story, one
family, wood framed dwelling, with attached garage; accessory shed;
all situated in "A" Residential Agricultural zone, with access to
Peconic Bay Blvd; a town maintained road.

fo!lows:
That I am the owner of certain premises known as 5700 Peconic Bay Blvd.,
Laurel, New York, designated on the Suffolk County Tax Map as District 1000
Section 128.00 Block 01.00 Lot 014.000.
That I submitted to the Southold Town Building Department an Affidavit
in support of my application for a pre-existing Certificate of Occupancy and
wish to supplement said Affidavit with this Affidavit to state that the
subject property also contain~ a shed prior to Apr1! of 1957. I request
that a pre-existing Certificate of Occupancy be issued for said shed as wel!
as the pre-exsiting house with attached garage.
