Tax Roll: Frances Majeski Occupied: Tenent, Barbara Baco Upon receipt of an application for a Pre-existing Certificate of Occupancy, I made an inspection of this one- story, framed dwelling. I was admitted to the building by Mrs. Baco and began this inspection at approximately 2:40 P.M. The building consists of one bedroom, living room, kitchen, and a full bathroom. The building is constructed on locust posts and is tightly skirted. Heat is furnished to the building by a floor -standing space heater, kerosene fired, located in the living room. The following Violations of the Housing Code, Chapter 52, Town of Southold, New York, were found: 1. Living Room - outside entry, no means of controlling light in room on entry; Article V, Section 52-56 B (2). 2. Bedroom - no door to provide privacy; Article II, Section 3-2---24--D (2) . 3. Kitchen - gas cooking range, no shut off valve at unit; Article V, Section 52-52 B (2). Outlet for refrigerator and ceiling fixture, current supplied to these fixtures by lampcord running through petition dividing kitchen and living room. This lampcord has pigtail plugged into outlet in living room wall; Article V, Section 52-56 A (1) 4. Bathroom - no moisture -resistant floor covering; Article II, Section 52-26 (4). Wall fixture, lamp cord providing cur- rent for wall fixture through petition to an outlet in the kitchen. Connection is made under the plate of the outlet; Article V; Section 52-56 A (1). Hot water heater, electric fired, two wires connecting, no shield and no ground; Arti- cle V, Section 52-56 A (1). 5. Exterior of building - southwest corner, siding of structure and structure itself below grade level, subject to elements & destruction from insects; Article III, Section 52-30 C and Section 52-31 B. Pole on northwest corner for electric service to building has been damaged by termites at the base and section at roofline is cracked and weakened, Con- duit that supplies the service line to the meter box is loose; Article V, Section 52-56 A (1) (2). This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in- formation required to prepare a certificate. C. Fees: 1. Certificate of occupancy $5.00 2. County of Suffolk) as: State of New York) Frances Majeski, being duly sworn deposes and says: That she resides on King Street, New Suffolk, and is the owner of the above premises located on 2nd Street, New Suffolk, N.Y. bounded on the North by Onody, East by Renick, South by Victoria and West by 2nd Street per copy of attached survey. That pursuant to a copy of the attached variance granted to her predecessors in title, the one story frame house situated on the property has been approved for occupancy on the basis of being continually inhabited for many years since its construction, on information and belief in the early 1930's. That the dwelling is in good condition, since she personally resided in the dwelling at one time, and. is now occupied by a tenant. That she makes this affidavit in support of an application to the Building Department to issue a Certificate of Occupancy on the premises.
