APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic featu res.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

IN RE: PREMISES, 1450 NAUGLES DRIVE CERTIFICATE OF OCCUPANCY ON
MATTITUCK, NEW YORK, A/K/A AS DISTRICT 1000, pRE-EXISTING DWELLINGS
SECTION 09.900-BL 05.00-LOT 022.000~ TAX MAP.
WILLIAM D. LOWE, being duly sworn, deposes and says:
1. I am the owner of the above premises and make this affidavit in
support of the issuance of a Certificate of Occupancy by the Building Depart-
ment of Southold Town.
2. Title was received by me from Carl G. Davidson, a builder, on
March 30, 1981 and recorded in the office of the Clerk, Suffolk County in
Liber 8985 at cp. 96. The completion of both buildings existed prior to
April, 23, 1957.
3. The attached survey by Smith & Young, dated March 25, 1981 and
guaranteed to me shows, among other items, a 1 story frame dwelling, breeze-
way and attached 2 car garage (]450 Naugles Drive). Also shown is a 1 story
frame dwelling to the South on a plot of land 100' X 200'~1450A Naugles Drive
4. There have been no inspections by any municipal authority made prior.
