The permittee is required to provide evidence that the non-disturbance buffer condition of the Trustee permit has been recorded with the Suffolk County Clerk's Office as a notice covenant and deed restriction to the deed of the subject parcel. In order to file this,the following are needed: 1- Completed template (this has been attached to the email along with a sample template to help assist you in filling it out). 2- Copy of the latest deed to this property is required. 3- A photocopy of the stamped approved survey plans from the Board of Trustees,write "Exhibit A" somewhere visible on the plan. The photocopy cannot be any larger than 8%"x14" Legal size paper so you will have to shrink down your plan. The Ink Spot on Boisseau Avenue,Southold (right behind Wayside Market) can shrink down large paper to what you need. 4- Blank check for fees to process the addition of the paperwork to your deed. Bring all of these items to the Suffolk County Clerk's Office in Riverhead at 310 Center Drive in Riverhead. Once they have given you the recorded covenants and restrictions, please forward us a copy. Once we obtain the copy,the office can issue the Certificate of Compliance. If you like,you can submit in writing the completed template to our Assistant Town Attorney, Damon Hagan,to review the draft prior to submission to the County-just be sure to include a copy of the permit, stamp approved plans, and contact information for him. Box1179 Southold, N.Y. 11971 FJAN -19 2017 Re: amendment to permit 8861 Southold]own 1"/2/17 Board of Tr Dear Trustee Board Members As per the instructions given to me via a telephone call (Diane) to your office, I would like this letter to serve as a request to amend a permit (8861) which I received already in August of 2016 . As required for an amendment by your agency I am enclosing a copy of the letter of approval from DEC in this regard,and 3 copies of the requested new plan on surveys and my $50 check. My dock builder feels that the amendments that I am requesting would make for a better construction and a better finished product. Therefore,without changing the position on the property or the length, width, or height of the project, I would like to request that instead of 5 inch pilings 5 feet apart I would like to amend the permit to be 8 inch pilings 8 feet apart. In addition, instead of thru-flow 60% light penetration decking on the project I would request that the decking be amended to ipe natural wood decking. Please note that there is no vegetation on my property nor on my neighbors property to my right or to my left. 1 would hope that you find these amendments acceptable, and, not affecting the original permit substantially.
