APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted • to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Mildred Grathwohl Long, being duly sworn deposes and states as follows:
(1) That I am the owner of certain premises located atIZ5 Maiden Lane, Mattituck,
New York, designated on the Suffolk County Tax Map as District 1000,
Section 104 Block 1 LotlO.
(2) That located on said premises are the following buildings:
A) A main house consisting of a separate upstairs rental apartment having
its own bath and cooking facilities and two separate and distinct living
units downstairs, one of which I have occupied for my own residence and the
other which is a separate rental unit, each having its own kitchen and bath
facilites and having its own separate entrances.
B) A separate rental cottage having its own bath and cooking facilities
and separate well and sanitary disposal system and electric meter.
C) The metal storage building for which a Certificate of Occupancy Z6448
was issued under building permit #6679Z.
(3) That the three separate living facilities in the main house and the separate
rental cottage all pre -date April 1957 and the rental facility in the
cottage and the upstairs and downstairs rental units in the main house have
been continuously occupied since prior to April 1957 by rent paying tenants

(4) I submit this affidavit in support of my application for a pre-existing
Certificate of Occupancy for a three family house and separate rental cottage.
In my opinion, all of said living units are habitable units.
