cable provisions of said Code.
IT IS FURTHER CERTIFIED that, based upon information presented to
the Building Inspector's Office, the occupancy and use for which this Certifi-
cate is issued is as follows: Property contains a one story, wood
framed; one family dwelling, with attached garage and patio.
Property also has a fence across the front of the patio and an
accessory building in the rear yard. Parcel situated in the A -
Residential -Agricultural Zone w/access to Three Waters Lane.

Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

FREDERICK J. TEDESCHI, an attorney duly licensed to
practice law in the State of New York, having an office at
218 Front Street, Greenport, New York, does hereby affirm
under penalties of purjury as follows:
1. I am the attorney for Edward A. Linker and Doris E.
Linker, his wife, who own the premises at 750 Three
Waters Lane, Orient, New York. The said premises
are known on the County Tax Map as Dist. 1000, Sect.
015, Block 06, Lot 023. They are also known on the
Subdivision map of Orient -By -The -Sea as Lot # 12.
2. I have discussed the existing structures on the pre-
mises with my clients and they inform me that they
include the structures as shown on the attached sur-
vey of Roderick Van Tuyl, P.C. And are in habitable
condition.
3. They further inform me that all of the structures and
uses therin have be(
