Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).' 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. For existing buildings (prior to April 1957), Nonconforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of p:operty showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. STATE OF NEW YORK) SS.: COUNTY OF SUFFOLK) WILLIAM WICKHAM, residing at (No #) Old Harbor Road, Cutchogue, New York, being duly sworn, deposes and says that he is the Executor under the Last Will & Testament of HELEN M. KURTH, who died a resident of Suffolk County on January 24, 1986; that at the time of her death she was the owner of the property on the East side of Marlene Lane, Mattituck, New York, as shown on the attached survey. The premises consists of a one-story frame dwelling, a frame gargage and a frame shed. Deponent has been the attorney for HELEN M. KURTH for many years and is familiar with the property. The property was purchased from WILLIAM F. ABITZ, the builder, by Mrs. Kurth, then HE?.~N M. KROEGER by deed dated June 19, 1953, and recorded in Liber 3535, Page 33 on 6/22/53. The so-called shed was originally a studio for Mrs. Kurth who was a well known artist and then used by Otto Kurth. Recently however, the building has been used as a tool shed. To deponent's knowledge there have been no changes or additions made to the buildings that would require a Building Permit or Certificate of Occupancy, since 1957.
