This application must be filled in typewriter OR ink, and submitted m ~ to' the Building Inspec- tor with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal). 3. Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa- tions, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5. Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. KUJAWSKI, being duly sworn, depose and say: That they are the owners of premises at 177 Sound Avenue, Mattituck, New York (Suffolk County Tax Map No. District 1000, Section 120.00, Block 03.00, Lot 001.000). That they acquired title to this parcel by deed dated July 14, 1969 and which was recorded in the Suffolk County Clerk's Office on July 17, 1969 in Liber 6586 Page 239. -The premises was previously owned by ~ohn Kujawski and Dorothy Kujawski prior to 1900 and has been in the family since that time. Located on the property is a two-story frame dwelling in ood condition and a barn, garage, chicken coop and corn crib which are in an old and dilapidated condition. The application is made for the purpose of obtaining a pre-existing Certificate of Occupancy from the Building Department of the Town of Southold. / Stanley M.
