Instructions
A. This application must be filled in typewriter OR ink, and submittedp+ieete•to the Building Inspec-
torwith the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equall.
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, acertificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 19571, Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa•
tion required to prepare a certificate.

STATE OF NEW YORK )
COUNTY OF SUFFOLK )
Alfred J. Kron being duly sworn desposes and says
that he is the owner of property located on the South side
of Main Road, Route 25, Mattituck, New York known as Kron
Galleries (Dist. 1000, Sec. 122, Blk. 3, Lot 9).
Based on the information available to me, it appears
that a building permit for a rear extension was issued under
permit /f2193Z and that a Certificate of Occupancy was issued
for said extension on April 14, 1964 to the then owner, Katherine
Nutley, for use as a private one family and business use.
Subsequent thereto another building permit was issued
under /I2924Z for a small addition to the main building on the
east side of the premises which was completed and for which
another Certificate of Occupancy was issued.
As far as the main building is concerned, there never
was a Certificate of Occupancy issued but as far as I know,
the building existed prior to April 23, 1957 and has been
continuously used since that time in accordance with the exist-
ting Certificate of Occupancy for the addition to the house,
namely a private one family dwelling and business use.
I have owned the building for over 10 years and it
is in good condition and livable.
