This application must be filled in by typewriter OR ink and submitted to the building inspector with the following: for new building or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2. Final Approval from Health Dept. of water supply and sewerage-disposal (S-9 form). 3. Approval of electrical installation from Board .of Fire Underwriters. 4. Sworn statement from plumber certifying that the solder used in system contains less than 2/10 of 17> lead. 5. Commercial building, industrial building, multiple residences and similar buildings - and installations, a certificate of Code Compliance from architect or engineer responsible for the building. 6. Strang Architect Main Road, P.O, I3os 1412, Southold, NY ll971 ~716~ %fJ-i~~c7J March 14, 1992 Mr. Gary Fish, Building Inspector Town of Southold Main Road Southold, New Xork 11971 Re: Roof Condition, Premises #10tl©-114-11-12, Main Road, Mattituck Dear Gary: In accordance with our recent telephone conversation, a field evaluation of the existing roof condition of the above referenced premises was made on Thursday, March 12, 1992. The following are my findings: The roof consists of five (5) framed bays, each approximately 17', framed with 2' x 10' roof rafters, @ 16" o.c., with exception to the northerly most bay, which has an increasing span from 17' to 20', due to the angulated front wall. This particular bay is framed with 2' x 10's, @ 16" o.c., on the westerly third (shorter span) and 3' x 1GJ's, @ 16" o.c., on the easterly two-thirds (longer span). The roof sheathing is made up of 1 x 8, tongue and groove roofers, over which a built-up, flat roof was applied. Inasmuch as this roof has little or no pitch, rainwater has ponded over the years, causing a breakdown of some of the built-up roofing material, as evidenced by numerous roof leaks. It is my opinion that although the roof structure does not appear to be in jeopardy of failing at the present time, measures should be taken to eliminate the leaks, as the sheathing, and several roof rafters in each bay are begin- ning to deteriorate, due to water saturation. Page Two March 14, 1992 Mr. Gary Fish, Building Inspector It is my recommendation that the roof be stripped and sheathing removed, at which time each rafter can be adequately assessed and replaced as necessary. At that time, new sheathing and roofing membrane will be installed. Consideration should be given to ei- ther providing a pitch on this roof or installing adequate drain- age for the removal of rainwater.
