1. Final survey of property with accurate location of all buildings, property lines,
streets> and unusual natural or topographic features.
2. Pinal Approval from Health llept. of wafer supply and sewerage-disposal(S-:' Eorm).
3. Approval of electricarl installation from board of Fire Underwriters.
4: Sworn statement from 'p lumber certifyin}; that Che solder used in system conCairs

5. Commercial building, Industrial building, multiple residences and similar buildings
and installations,~\a certificate of Code Compliance from architect or engineer
responsible for the building.
G. Submit Planning Board Approval of completed site plan requirements.
6. For existing buildings (prior to April 9, 1957) non-conforming uses, or buildings and
"pre-existing" land uses:

unusual natural or topographic features.

says:
That I reside at Driftwood Cove Condominiums, Greenport, New
York.
That my family has owned property along Sage Boulevard,
Greenport, New York since approximately I~~ec JJ~
That I am fully familiar with the premises currently owned
by Joseph and Marie Knizak located at 1490 Sage Boulevard,
Greenport, New York, which parcel bears Suffolk County Tax Map
No. 1000-053.00-05.00-011.002.
That, to my personal knowledge, since the early 1950's,
there existed on the aforesaid premises the two sheds located
along the easterly property line of the Knizak parcel and the
garage located near the westerly line of the Knizak parcel, as
more fully shown on the copy of survey map annexed hereto.
That thereafter, in or about 1962, said premises were sold
to John and Mabel Higgins, who shortly thereafter built a house
thereon. In approximately 1980, Mr. and Mrs. Higgins sold the
subject premises to the Knizaks.
That as a result, the two sheds and the garage currently
existing on the subject premises have been located on the parcel
since the early 1950's, which is prior to zoning in the Town of
Southold. _
