APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in duplicate to the Building Inspec-
torwith the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal-(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, acertificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
6. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusua~ natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

FRED W. KAELIN, residing at (No Alvah's Lane,
Cutchogue, Town of Southold, New York, being duly sworn,
deposes and says:
I am the owner of premises, located on the s/s of Main
Road, Cutchogue, New York, Suffolk County Tax Map #1000-
102.00-06.00-006.000, as shown on the enclosed survey. I
purchased this property from RUSSEL A. BROWN and wife in two
parcels, one on April 11, 1952, Liber 3341, Page 163 and one
on November 2, 1953, Liber 3608, Page 568.
Deponent further says at the time of the purchase of
the premises, the building was fully erected and had been
built some years prior to that time; that it contained two
(2) separate offices on the main floor and an apartment on
the second floor; that the offices and apartment have been
in use as such since deponent's acquisition and are now in
such use.
To my knowledge, there have been no changes or
alterations made to the buildings that would require a
Building Permit or Certificate of Occupancy, since 1957.
I make this application to the Town of Southold for a
Certificate of Pre-Existing Use for the two-story frame
building, with two offices and an apartment.
