Please be advised that according to our records and inspection report, the above
premises were in an un -heated, gutted, non -habitable condition at the time of
inspection.
An observation on this date shows the premises to have four electric service
meters, an extension at the rear, renovation of accessory structure and the
house is occupied.
These conditions are evidence of several violations of Town Code and NYS
Building and Fire Prevention Code.
Please contact this office immediately to discuss the legal remedy to the
conditions at this location. An inspection by our inspector is requested to
determine the exact conditions within the dwelling.

IRENE KOWALSKI, being duly sworn, deposes and says:
That she is the daughter of Stephna Zazecki a/k/a Stefni
Zazecki, and has lived in Southold for more than 40 years and is
fully familiar with the tax map parcel identified as District 1000,
Section 069.00, Block 06.00, Lot 007.000.
The above parcel was my residence for many years and is
within one mile of my present residence. Until the death of
Stephna Zazecki a/k/a Stefni Zazecki, my mother, this residence was
used continuously from prior to 1957 until the date of her death as
a two family dwelling. (Date of death January 16, 1989)
It has not recently been used as a two family dwelling
solely because of her death and the inability of her estate to sell
this parcel in a declining market.
I make this affidavit knowing it will be relied upon by
the officials of the Town of Southold with respect to issuance of
a two family dwelling certificate for the subject parcel.

Dear Mr. Boufis:
I stopped by to discuss this property with you but you advised
me that you were having a meeting tomorrow with regard to this
property and I believe that the enclosed information may be
helpful.
Mr. Thomas C. Jerome applied for a pre-existing certificate of
occupancy on July 31, 1996 for a two-family residence. On
September 11, 1996 the building department issued a pre-existing
certificate of occupancy for non -habitable one family dwelling with
accessory buildings.
Enclosed are four separate statements, Irene Kowalski, the
daughter, and Gayle Bukmier, Alfred H. Goldsmith and Louis Baker
long-time neighbors who are personally familiar with the premises
and which state that this property is a pre-existing two-family
residence. The house was owned and occupied by Stephna Zazecki as
a two-family residence, with two kitchens and two separate living
quarters. Mrs. Zazecki died on January 16, 1989 and the estate
took several years to resolve. Thereafter the property was placed
for sale, the estate advertised the property as a two-family
residence. Thomas Jerome purchased the property as a two-family
residence. Mr. Jerome painted and cleaned up the property
(nonstructural improvements), retained the two kitchens and
continued the two-family use.
There is no question that this house is a two-family
residence, it is my understanding that you may have been
misinformed by someone to the contrary. We would like to clear
this up with your office. If this matter cannot be resolved with
your office, than we are prepared to appeal your determination to
the Zoning Board of Appeals.
