garage, property is zoned "A" Residential -Agricultural and has access
to Gillette Drive.

tor with the following; for new buildings or new use-.
1. Final survey of property with accurate locatioi of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board o, Fire Underwriters.
4. Commercial buildings, Industrial buildings, Mcl-iple Resicences and similar buildings and installa-
tions, a certificate of Code compliance from -'r A c"ite, nr Fn(-,�neer responsible for the building.

Suffolk, State of.New York, and designated on the Suffolk County Tax
Map as District 1000, Section .038, Block 04, Lots 007 and 008, and am
familiar with the condition of same.
That the structure situated thereon has been designed,
arranged and physically oc cupied as a single family dwelling;
That at this time, the premises is occupied;
That the condition of the existing structure on the premises
is good;
That I make this affidavit in the knowledge that the Building
Department of the Town of Southold will rely on the accuracy thereof
