WILLIAM WICKHAM, residing at 1150 Harbor Road, New
Suffolk, New York, being duly sworn, deposes and says:
I am an attorney with the firm WICKHAM, WICKHAM &
BRESSLER, P.C., Main Road, Mattituck, New York. Deponent
makes this affidavit for HELEN G. HORTON, who is the owner
of the premises located at New Suffolk on the South side of
New Suffolk Avenue, Cutchogue, New York, Suffolk County Tax
Map #1000-117.00-09.00-009.000, as shown on the enclosed
survey.
The house and garage on the premises were built in 1920
by Goldsmith & Tuthill for Mrs. Horton and her husband,
Gilbert Horton, who was the bookkeeper for the firm, and the
house was given to them as part of his employment.
The Hortons lived on the property from 1920. Mr.
Horton died in 1971, and Mrs. Horton continued it as her
residence. The property was conveyed to them in 1956 by
Ruth Tuthill Houston. To my knowledge and belief, no
alterations were made to the premises that would require a
Certificate of Occupancy.
I make this affidavit to the Town of Southold for a
Certificate of Pre -Existing Use for the residence and
garage.

Instructions
A. This application must be filled in typewriter OR ink, and submitted nu. to the Building Inspec-
tor with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.
C. Fees:
