3% The purpose of this application: Redriving or replacing o~enwork
dock or ~penwork pier piling, and mooring pil8s, removal of damaged
pi~ing, repair, resetting or replacement of floating docks, ramps,
o~decking, all necessitated by ice and storm damage resulting during
1984-1985:-w~nter.
SPECIAL CONDITIONS
1. No dredging or filling is perm ted.
2. Repairs to existing
timbers without change
struc~ure~ shall be limited to replacement of

3. No repair or replacement of bulkheads ~s permitted under this permit'.
4. NO freshwater or tidal wetlands are to be disturbed under any
circumstances.
~5. A yellow permit s~gn provided by this office shall be displayed on
the project a~ all times while under replacement.
6. Original permit numbers (~_high) must be permantlj displayed on
project.

transmitting an application for an Ice Damage Permit
per your request.
Please complete same, and return to this office with
copies of your D.E.C. and Army Corps Permits.
Additionally, I have attach~d section 97-12 of Chapter 97
(Wetlands) for your information. If you have any questions or
