APPLICATION FOR CERTIFICATE OF OCCUPANCY
Instructions
A. This application must be filled in typewriter OR ink, and submitted in triplicate to the Building
Inspector with the following; for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and
unusual natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and
installations, a certificate of Code compliance from the Architect or Engineer responsible for
the building.
S. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non -conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of property showing all property lines, streets, buildings and unusual natural
or topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in-
formation required to prepare a certificate.
C. Fees:

HOUSING CODE INSPECTION
November 10, 1977

Upon receipt of an application for a Pre -Existing Certificate
of Occupancy, I made an inspection of building at this location.
I was admitted to dwelling by Robert Schorr of the Jack Driscoll
Real Estate Agency of Mattituck, N.Y., who accompanied me during
this inspection that began at approximately 2:00 P.M.
The building is a one-story wood framed structure con-
structed on a concrete block foundation. Building contains a
hallway that provides front entry and access to utility room
and one -car attached garage on west side of building. Rooms
on west side of hallway consist of a full inside bathroom, a
kitchenette and two rooms with large passage way in partition.
Rooms east of hallway consist of a full inside bathroom, a room
with kitchen cabinets and kitchen sink, a dining room, a room
with closet facilities and built-in chest of drawers (this room
provides access to living room on east side of building.)
Living room is constructed over crawl space of foundation
with access door on exterior of foundation. Balance of building
is constructed over crawl space with access door in floor of
hallway.
The following violations of the Housing Code, Chapter 52,
Code of the Town of Southold, New York, were found:
Alcove - off room west side of hallway used for sleeping
contains closet facilities for clothes storage) no door
way to provide privacy. Window in alcove, opening less
than 10% of floor area and does not provide suitable
opening for emergency egress. Area of room less than
required for habitable room, Article V, Section 52-22 A 1,
Section 52-24 C 1-3 & D 2.
Bathroom - east of hallway, inside, no mechanical venti-
lation, Article V, Section 52-26 B 4 b.
Bedroom - connects dining room and living room, Article V,
Section 52-24 D 2.
Electric - motor water pump, pigtail wired, Article V,
Section 52-56 A 1-5.
Inspection completed at approximately 2:20 P.M.
