VIRGINIA R. HINSCH, residing at Fifth Street, New Suffolk, New York being duly
sworn deposes and says that she is the daughter of George Hinsch and Edna Hinsch, deceased,
prior owners of property located at Fifth Street, New Suffolk, New York (SCTM#1000-117-10-14).~L-
That this affidavit is being submitted in support of our application of a Pre Existing
Certificate of Occupancy.
That the property was originally owned by my grandfather, CHARLES HEMPEL, who
purchased the property in 1936. He also was the owner of the adjacent property now owned
by Andrew Goodale.
In 1965 my grandfather conveyed his house as a gift to my parents, George Hinsch and
Edna Hinsch.
As far as I can determine the house was built in 1883 and there have been no
substantial changes made at the house since the time my parents acquired title in 1965.
As long as I can remember, the main portion of the house was always used as a one-
family house and the unheated portion of the house of the porch has always been used as a
summer rental unit and has been so used as such since 1936.
WHEREBY it is respectfully requested that a Pre Existing Certificate of Occupancy be
issued in accordance with the above.

APPLICATION FOR CERTIFICATE OF OCCUPANCY
This application must be filled in by typewriter OR ink and submitted to the building
inspector with the following: for new building or new use:
1. Final survey of property with accurate location of all buildings, property lines,
streets, and unusual natural or topographic features.
2. Final Approval from Health Dept. of water supply and sewerage-disposal(S-9 form).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Sworn statement from plumber certifying that the solder used in system contains
less than 2/10 of 1% lead.
5. Commercial building, industrial building, multiple residences and similar buildings
and installations, a certificate of Code Compliance from architect or engineer
responsible for the building.
6. Submit Planning Board Approval of completed site plan requirements.
For existing buildings (prior to
"pre-existing" land uses:

Accurate survey of property showing all property lines, streets, building and
unusual natural or topographic features.
A properly completed application and a consent to inspect signed by the applicant.
If a Certificate of Occupancy is denied, the Building Inspector shall state the
reasons therefor in writing to the applicant.
