Div.: Laurel Park #212, Lots 13 & 14 Tax Roil: Frank Henjes & wf. Upon receipt of an application for a Pre-Existing Certificate of Occupancy, I made an inspection of this dwelling. I was admitted to building by Mr. Henjes and began this inspection at approximately 1:50 P.M. Additions to dwelling were made under Building Permit #2360Z dated April 17, 1964, and Building Permit #3644Z dated September 26, 1967, to which Certificates of Occupancy were issued, #Z2038 and #Z3052. The pre-existing portion of building is a wood framed structure constructed on a cement block foundation providing a partial cellar. First floor contains a kitchen, living room, bathroom, and bedroom. Second floor has a bedroom and sitting room at top of stairway. The following violations of the Housing Code, Chapter 52, Town of Southold, New York, were found: Kitchen - gas cooking range, no shut off valve at unit, Article V, Section 52-52 B 2. Cellar - gas fired hot water heater, no shut off valve at unit, Article V, Section 52-52 B 2. Open electric junction box over doorway entry to cellar, Article V, Section 52-56 A 1. Second floor used for habitable space, head room is less than 7'6", a violation of the Zoning Ordinance, Chapter 100, Code of the Town of Southold, New York, Article I, Section 100-13, "Floor Area Livable." This application must be filled in typewriter OR ink, and submitted in DUPLICATE to the Building Inspector with the following; for new buildings or new use: 1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual natural or topographic features. 2.Final approval of Health Dept. of water supply and sewerage disposal (S-9 form or equal). 3.Approval of electrical installation from Board of Fire Underwriters. 4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installations, a certificate of Code compliance from the Architect or Engineer responsible for the building. 5.Submit Planning Board approval of completed site plan requirements where applicable. B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing" land uses: 1. Accurate survey of property showing all property lines, streets, buildings and unusual natural or topographic features. 2.Sworn statement of owner or previous owner as to use, occupancy and condition of buildings. 3. Date of any housing code or safety inspection of buildings or premises, or other pertinent in- formation required to prepare a certificate. C. Fees: 1. Certificate of occupancy $5.00 2.
