3, That this Permit is valid for a period of t 2 mos. which is considered to be
sufficient time required to complete the work involved, but should circumstances warm% request
for an extension may be made to the Board at a Eater date.
42 That this Permit should be retained indefinitely, or as long as the said Permhtee wishes
to maintain the stmctore or project involved, to provide evidence to anyone concerned that autd2-
orizarinn was origiwaily obtained.
5 ~ That the work involved will be subject to the inspection and approval of the Board or
its agents, and non-compliance with the provisions of the originating application, may be cause for
revocation of this Pen'nit by resolution of the said Board.
6. That there will be no unreasonable interference with navigation as a result of the work
herein authorized. ~
7. That there shall be no interference with the right of the public to pass and repass along
the beach between high and low water marks.
8. That if fucere operations of the Town of Southold require the removal and/or alterations
in the location of the work herein authorized, or if, in the opinion of the Board of Trustees, the
work shall cause unreasonable obstrucaon to free navigation, th~ said Permit-tee will be required~
upon due notice, to remove or alter this work or project herein stated without-expenses to the Town
of Southold.
9. That the said Board will be notified by' the Permktee ot the completion of the work auth-
orized. (See tear off sheet.)
10. That the Permittee will obtain all other permits and consanu that may be required sup-
plemental to this perm/t which may be subject to revoke upon failure to obr.;- same.

Dear Mr. Kujaw~ki,
Pursuant toy our letter concerning our permit for a dock on Deep Hole Creek
in Mattituck, please be advised that a d~kwas installed in May 19~8 by
Mr. J. Frohnhoefer of Water Thrills Inc.. Prior to the installatic%a I
requested that he apply for a perm/t on my behalf and he advised that he
had checked Townof Southold records which showed that a dock had been
installed byprevious o~ers ~nd that no application was necessary. He
said that he noted on the record that the deCk w~s re-installed as of May 1978.
I never was given a copy of said notatic~ or new permit, believing that every-
thing %fas done properly.
Assuming that allis not in order, I mn enclosing a new application for your
review and approval.
