SCM # 1000-061-02-05, -06, & -07 Dear Mr. Hinderman, Item B (2) of the application for a Certificate of Occupancy requires a sworn statement as to prior use, occupancy and condition of the buildings involved, to wit: Building shown on Parcel 1 of attached survey: Prior use with approximate dates: Doctor's residence and office. Intermittent family use since then, with full-time live-in maid. Occupancy with approximate dates: Dr. & Mrs. Harry E. Stevens residence until deceased. Live-in maid, Lilley Johnson, after that occurrence until her recent demise. Condition of building(Good) (Fair) (Poor) Buildings shown on Parcel 2 of attached survey: Prior use of 1 story frame building with approximate dates: Insurance office and store and studio until approximatively August l, 1982. Intermittent use since then by family in connection with estate settlement. Occupancy of 1 story building with approximate dates: Arthur Gagen, insurance and real estate; J.B.
