Instructions
A. This application must be filled in typewriter OR ink, and submitted piiee+e to the Building Inspec-
tor with the following;for new buildings or new use:
1. Final survey of property with accurate location of all buildings, property lines, streets, and unusual
natural or topographic features.
2. Final approval of Health Dept. of water supply and sewerage disposal—(S-9 form or equal).
3. Approval of electrical installation from Board of Fire Underwriters.
4. Commercial buildings, Industrial buildings, Multiple Residences and similar buildings and installa-
tions, a certificate of Code compliance from the Architect or Engineer responsible for the building.
5. Submit Planning Board approval of completed site plan requirements where applicable.
B. For existing buildings (prior to April 1957), Non-conforming uses, or buildings and "pre-existing"
land uses:
1. Accurate survey of peoperty showing all property lines, streets, buildings and unusual natural or
topographic features.
2. Sworn statement of owner or previous owner as to use, occupancy and condition of buildings.
3. Date of any housing code or safety inspection of buildings or premises, or other pertinent informa-
tion required to prepare a certificate.

Upon receipt off application for pre-existing certificate of occupancy
I made an inspection of this one and one half story framed dwelling and found
the following violations of Chapter fifty two ( 52) Housing code of the Town
of Southold.
I was admitted by Mrs Guilbert and was accompanied by Mr Guilbert and
Mr. J. Baker on the inspection which was begun about 9:05 AM.
The first floor consists of front enterance vestibule, utility room,
dining room, living room, full bath room, kitchen and enclosed rear porch,
access to partial cellar through trap door in porch floor.
1 . Living room: Lamp cord outlets on north wall baseboard Art V Sec 53-56 Al
2. Rear porch: no means oflight controll at point of entry " " 52-56 B'
3. Cellar(partial) : water pump electrical connection & hookup
needs junction box & connected to pressure
switch wnclosed If " 52-56 Al
4. Second floor consists of four rooms, three used as sleeping
quarters and one storage room.
5. Storage room: lamp cord outlet " " 52-56 Al
6. Bedroom on north west used as access to bedroom on N/East " " 52-24 D2
Heat is furnished to all rooms from gas fired hot water
heater in utility room.
Exterioft and open space are in good condition
Inspection completed about 9:30 A.M.
