Town Trustees at a regular meeting held December 3, 1980.
RESOLVED to reeommend to the Southold Town Board
approval of the wetland application of Carol Galgano to
erect catwalk over wetlands at private property of the
applicant at Mattltuck Creek with the condition that the
catwalk be kept in line with the existing catwalk and be kept
above the grass.

Dear Sirs:
We respectfully request that the above permit be extended
beyond its original expiration date of 12/3/81. We make
this request on behalf of our client, who has contracted
to purchase property from Carol Galgano. The subject
property is a vacant parcel located off Westphalia Road,
Mattituck, and is shown on the enclosed photocopy of
survey. The desired construction would extend into Mapes
Creek from the 15 foot right of way running along the
westerly side of the premises.
As our client expects to take title to the property on
September 10, 1982, would an extension of 12 months from
that date be reasonable to the Board?
I am enclosing a copy of the original permit for your
reference.

A. This application is to be completely filled in by typewriter or ink and submitted to the Town
Clerk in quadruplicate, accompanied by writter~ permms~on from the owner of the property if not
the same as the applicant.
B. Pilot Plan must be drawn to scale on the reverse side of ~his application showing the location
of property, the names and locations of owners of adjoining properties, and a detailed description
of proposed project. (Note: Copies of the Pilot Plan may be made by a copy machine and attached
to the Permit Application.)
C No operation shall be initiated by the applicant until all permits that are required are issued.

work is completed must be given, including a survey if required.
E. if permit is being sought for a more sizeable excavation l+~n the construction of a private
single dock or jetty, then this application must be accompanied by a survey and topographical map
certified by a registered land survey or Professional Engineer. 'Fha horizontal control of survey
shall be based upon an approval local coordinate system. The vertical cor~trol for elevation and sound-
Ings shall be based upon United States Coast and Geodetic Survey and/or United States Geological
Survey datum.
F. A statement must accompany the application describing any known pnor operations conducted
on the premises 'n question end whether any prior licenses or permits have been issued ~o erect
structures or to dredge or deposit fill on said premises and whether om/ such permits or licenses
were ever revoked or suspended by a governmental agency.
G. A filing fee of $25.00 shall accompany the application. This filing fee includes one inspection:
there is a $5.00 fee for each additional inspection.

the provisions or Article II, Section 'J~l~,'subdivisions (~J and {~'where it finds that nature of the
proposed operations are such that ~he requirements of such provisions are not necessary for a
