May 8, 1984 adopted a resolution denying your Wetland Application
No. 175. This denial is without prejudice to any future applica-
tion.
The Board also adopted a resolution, copy of which is enclosed,
declaring this a Type I ,action and requesting a Draft Environmental
Impact Statement.

THE FOLLOWING RESOLUTION WAS ADOPTED BY THE SOUTHOLD TOWN BOARD
AT A REGULAR MEETING HELD ON MAY 8, 1984:
WHEREAS, Ferucio Frankola has heretofore applied' to the Southold
Town Board for a permit under the provisions of the Wetland Ordinance
of the Town of Southold, for permission to dredge an existing dredged
boat basin at the south side of Glenn Road, off Goose Creek, Southold,
New York.
NOW, THEREFORE, IT IS RESOLVED AS FOLLOWS:
1. That pursuant to the provisions of Article 8 of the Environmental
Conservation Law, Part 617 of Title 6 of the New York State Code, the
Southold Town Board, as lead agency, does hereby determine that the
,action proposed is a Type I Action and is likely to have a significant
effect on the environment.
2. That the Town Clerk shall file and circulate such determination
as required by the aforementioned law, rules and, code.
3. That the Town Clerk immediately notify Ferucio Frankola of
this determination, and further request said applicant to prepare a Draft
Environmental Impact Statement, all in accordance with said law, rules
and code.

INSTRUCTIONS FOR PERMIT
A. This application is to be completely filled in by typewriter or ink and submitted to the Town
Clerk rn quadruplicate, accompanied by writter' perm ss~on from the owner of the property if not
the some as the applicant.
B. Pilot Plan must be drawn to scale on the reverse side of this application showing the location
of property, the names and locations of owners of adjoining properties, and a detailed description
of proposed proiect. (Note: Capias of the Pilot Plan may be made by a copy machine and attached
to the Permit Application.')
C. No operation shall be initiated by the applicant until alt permits that ore required are issued.
D. A detailed statement of the rehabilitation and proposed condition of the premises after the
work is completed must be given, including a survey if required.
E. If permit is being sought for a more sizeable excavation than the construction of a private
single dock or jetty, then this application must be accompanied by a survey and topographical map
certified by a registered land survey or Professional Engineer. The horizontal control of survey
shall be based upon an approval local coordinate system. The vert'ical control for elevation and sound-
ings shall be based upon United States Coast and Geodetic Survey and/or United States Geological
Survey datum.
F. A statement must accompany the application describing any known prior operations conducted
on the premises in question and whether any prior licenses or permits have been issued to erect
structures or to dredge or deposit fil~ on said premises and whether any such permits or licenses
were ever revoked or suspended by a governmental agency.
G. A filing fee of-$25.0O-shaH accompany the application. This filing fee includes one inspection,
there is a $5.00 fee for each additional inspection.
H. The Town Board upon request~c~f_~t~ applicant for a permit, may waive in whole or in part,
the provisions of Article tl, SectionT:~q~;/subdivisions (~ and ({~' where it finds that nature of the
proposed operations are such that the reauirements of such provisions are not necessary for a
proper consideration of a permit application.
