TERMS and CONDITIONS The Permictee Richard Fraker residing at Knollwood Lane, Mattituck N.Y. as of the consideration for the issuance of the Permit does understand a~d to the roi- parr p~4_be lowing: 1. Box 2124 Greenport, NY 11944 Re: Riced Fraker SCTM ~1000-107-6-1 Dear Mr. Costello: The following action was taken by the Board of Town Trustees during its regular meeting held on July 11,1991 regarding the above matter: WHEREAS, COSTELLO MARINE IH BEHALF OF RICHARD FPJd{ER applied to the Southold Town Trustees for a permit under the provisions of the Wetland Ordinance of the Town of Southold, application dated May 29, 1991 and, WHEREAS said application was referred to the Southold Town Conservation Advisory Council for their findings and recommendations, and, WHEREAS, a public hearing was held by the Town Trustees with respect to said application on July 11, 1991 at which time all interested persons were given an opportunity to be heard, and, WHEREAS, the Board members have personally viewed and are familiar with the premises in question and the surrounding area, and, WHEREAS, the Board has considered all the testimony and docUmentation submitted concerning this application, and, WHEREAS, the Board has determined that the project as proposed will not affect the health, safety and general welfare of the people of the town, NOW, THEREFORE BE IT RESOLVED THAT COSTELLO MARINE ON BEHALF OF RICHARD FRAKER BE AND HEREBY IS GRANTED PEP/~ISSION UNDER THE WETLAND ORDINANCE TO: Construct 4' X 90' timber dock, 30' X 20' alUminum ramp, 8' X 24' timber floating dock, (2) 2-pile dolphins. Located Knollwood Lane, Mattituck. This permit will expire two years from the date it is signed. Fees must be paid, if applicable, and permit issued within six months of the date of this notification. Two inspecnions are required and the Trustees are to be notified upon completion of said project. Please return to the Building Department for a determination on the need for any other permits which may be required for this project.
