INSTRUCTIONS FOR PERMIT
A. This application is to be completely ill[ed in by typewriter or ink and submitted to the Town
C1erk in quadruplicate, accompanied by written' permission from the owner of the property if not
the same as the applicant.
B. Pi~ot Plan must be drawn to scale on the -averse ~ide of this application showing the location
of property, the names and locations of owners of adjoining properties, and a detailed description
of oroposed pro]ear. (Note: Copies of the Pilot Plan may be mede by a copy machine and attached
lo the Permit Application.')
C. No operation shall be initiated by the applicant until all permits that ore required are issued.
D. A detailed statement of the rehabilitation anc proposed condition of the premises after the
work is completed must be given, including e survey if required.
E. If permit's being sought for a more sizeable excavation 'then the construction of o private
single dock or jetty, then this application must be accompanied by a survey and topogrophicc map
certified by a registered land survey or Professional Engineer. The horizontal control of survey
shall be based upon on approval local coordinate system. The vertical control for elevation and sound-
ings shall be based upon United States Coast end Geodetic Survey and/or United States Geological
Survey datum.
F. A statement must occomoany the application describing any known prior operations conducted
on the premises in question and whether any prior licenses or permits have been issued to erect
structures or to dredge or deposit fill on said premises and whether any such permits or licenses
were ever revoked or suspended by a governmental agency.
G. A fillng fee of $25.00 shall accompany the agolication. This Filing fee includes one inspection.
there is a $5.00 fee for each additional inspection.
H. The Town Board upon request,~orf. ~e applicant for a permit, may waive in whole oi in pert,
the provisions of Article II, Section '~; subdivisions (4 and (~where it finds that nature of the
proposed operations are such that the reauirements of such provisions ore nor necessary for o
proper consideration of a permit application.
